FALL 2011 Tryout Information / Schedule

Any student trying out for a team must have a current physical exam(done within the past year) on file with our school nurse.
There are 4 forms required.

1.  New Pre-Participation Questionaire (completed by parent and student)|

2.  New Pre-Participation Physician's Form (completed by physician)

3.  NJSIAA Steriod Waiver Form

4. NJSIAA Concussion Consent Form

5.  Fair Lawn High School Athletic Code of Conduct

6.  Emergency Information Profile

7. Updated Health History Form

After a student participates in a sport and wants to be eligible to participate in another sport in a NEW ATHLETIC SEASON, it is mandatory that the student fills out an Updated Health History form as well as an Emergency Information Profile.   These forms must be returned to the school nurse prior to the opening tryout.
